Incident Report

This Form is to be used for all incidents of serious illness, medical accident/injury, and summer camp rule violations in accordance with the Sunnybrae Bible Camp Summer Staff Handbook and Code of Conduct. Incidents are to be documented completely and 3 copies made, for the Office, Summer Camp binder, and for the Parent. 

Instructions:

• Inform the Camp Director immediately (regardless of time of day) and give a detailed explanation of the incident
• Fill out all sections of the this report completely
• Take needed actions as per the Sunnybrae Bible Camp Summer Staff Handbook, with Leadership assistance if needed
• Make THREE copies (for the Office, the Summer Camp binder and for the Parent to take)
• Follow up with Leadership and follow Next Steps with the Camper
